New Patient Guide

Everything you need to prepare for a first appointment with Eric Wexler, M.D., Ph.D.: how to schedule, what to bring, and what the initial evaluation covers.

Step by step

1. Request an appointment

Call the office at 310-744-1250, use the scheduling request form below, or submit a request through the online prospective patient portal. The office aims to see new patients within two business days, and often the same day when clinically needed.

2. Choose in-person or telehealth

Initial evaluations may be conducted in the Santa Monica office or by secure video for patients physically located in California. Telehealth follow-up is available for established patients.

3. Complete the intake packet

The packet has two parts: legally required consents and a medical, psychiatric and social history questionnaire. Completing both in advance keeps the first visit to 60 to 90 minutes and reduces its cost.

4. Gather your documents

Assemble the items in the checklist below. Records from prior psychiatrists, hospitalizations, neuropsychological testing and recent labs are especially useful.

5. Attend the initial evaluation

A 60 to 90 minute diagnostic evaluation covering current symptoms, history, medical background, prior treatment response, family history and goals of care.

6. Review your plan and schedule follow-up

You will leave with a working diagnosis, treatment options and a follow-up plan. Medication visits are 15 to 30 minutes; psychotherapy sessions are 45 to 50 minutes. If a specialized program is a better fit, a direct referral is provided.

What to bring

  • Completed new patient intake packet (consents plus history questionnaire)
  • Government-issued photo identification
  • Insurance card, if you plan to seek out-of-network reimbursement
  • A written list of all current medications, doses and supplements
  • Names and dates of prior psychiatric medications, and how you responded to each
  • Records from prior psychiatrists, hospitalizations or intensive programs
  • Recent laboratory results, EKG, or imaging if available
  • Neuropsychological or educational testing reports, if any
  • Contact information for your current therapist and primary care physician
  • For patients under conservatorship or with a legal matter, relevant legal documentation
